Data Collection Support for a Community Health Assessment

In collaboration with Sky Lakes Medical Center, Klamath Health Partnership, and Klamath County Public Health, this project implemented a community health survey to assess the collective wellbeing of the local population. The results of the assessment were used to plan and track progress of the Community Health Improvement Plan.
